BCIA 8016 — Request for Live Scan Service

Most Common

Standard form required for most employment, licensing, and background checks in California. Your requesting agency should pre-fill the ORI number.

BCIA 8016RV — Subsequent Arrest Notification

Rap-Back

Used for ongoing monitoring and rap-back enrollment. Required by agencies that need to be notified of future arrests.

FD-258 — FBI Ink Fingerprint Card

FBI / Federal

Required for federal agencies, immigration, out-of-state licensing, military clearances, and agencies that do not accept Livescan.
